I wish to enroll my child ['the Applicant'] for the 2008 Ottawa FURY Summer Camp ['the Program']. We agree to pay the full fees for the Program and understand that we are not entitled to any refund of fees if our child withdraws or is removed for any reason.
Acknowledgement, Waiver, Release and Indemnity
In consideration of the Ottawa Fury Soccer Club L.P. ('the Club') accepting the Applicant into the Program, I, the parent/guardian of the Applicant, for myself, the Applicant, any other parent/guardian of the Applicant, our heirs, administrators and assigns hereby (1) acknowledge that there are numerous inherent risks associated with the Applicant's participation in the Program and that I assume complete responsibility for those risks and for personal injuries, damage to property, accident of any kind, or death that may occur as a result of those risks even if the injury, damage, accident or death occurs in a manner that is not foreseeable (2) waive any claims, demand, damages, actions or causes of action (collectively called 'Claims' below) to which any of us may be entitled, arising out of or in consequence of the participation or attendance of the Applicant in the Program and any loss, injury or damage to my person or property or to the Applicant's person or property incurred while the Applicant attends or participates in the Program, and (3) releases the Club, its owner, shareholders, directors, servants, agents, sponsors, employees and any other person or organization assisting in the program (all of whom are collectively called the 'Program Providers') from such Claims, notwithstanding that any such loss, injury or damage may have arisen by reason of the negligence of all or any of the Program Providers.
I further agree, as parent/guardian of the Applicant, to indemnify the Program Providers against any Claims, notwithstanding that any such loss, injury or damage may have arisen by reason of the negligence of all or any of the Program Providers.
It is understood and agreed that the Club in its sole discretion is entitled to require the withdrawal of your child at anytime if it determines that no other sanction is appropriate in the circumstances (i) for conduct inconsistent with the Club's statement of values; (ii) for breach of the Club's rules and regulations, or (iii) in the event that the well-being of the Club would be prejudiced by your child's continued presence.
It is understood and agreed that the Club shall have permission to take video, still photographs and/or digital photographs featuring the Applicant at such times and at such places as the Club may designate, and to use the Applicant's name in connection therewith and shall also permit name, portrait, picture and voice to be used by the Club on all writings, audiovisual displays and websites relating to training, promotional, endorsement and publicity activities, and all rights to the foregoing shall belong exclusively to the Club
Acceptance of Enrolment Agreement Terms:
I accept and agree to the provisions of this Agreement. NOTE: By clicking on the "I have Read & Agree to the Liability Waiver" button you are agreeing to the foregoing provisions, including the general waiver and acknowledgement, which, among other things, limits the liability of the Club in certain areas.
